Please click the link                                                                                                                                                   the  special group plate name is CHILDHOOD CANCER LICENSE                                                                                                         PLATE and the name of the sponsoring organization is PRIMARY                                                                                                         CHILDREN'S HOSPITAL. On the required funds part on the                                                                                                                     bottom of the page, please put $25 on the line for initial                                                                                                                               contribution and $45 will be the total amount due.

In the memo line just put license plate registration. The check needs to be made out to Primary Children's Hospital. They will hold your check and registration application in an account until we reach 500 license plates. Then they will turn all the money and forms into the DMV to begin production of the plates. The DMV will mail your plate to you when they are done. Each year there will be an annual fee of $25 that will be given back to PCH for childhood cancer services and research.
